Description: The RN implements professional nursing practice centered on the patient as a whole person. The RN creates a caring environment focused on healing and wellness. Nursing practices are based on evidence, current research, hospital policies, and professional guidelines. Clinical decision-making incorporates all four phases of the nursing process, critical thinking, and creativity. Nursing excellence is promoted through helping relationships among nursing staff and across other disciplines. The RN provides direct patient care activities through nursing rounds, assessments, and treatments. The RN facilitates admission, transfer, and discharge activities. The RN provides patient and family-centered education and care coordination. The RN conducts comprehensive assessments in accordance with the RN scope of practice and hospital policies. The RN considers the patient’s culture, age, and developmental level during assessments. The RN identifies patient risk factors and obtains health information from family as necessary. The RN collects both objective and subjective data to inform patient care. The RN develops a problem list prioritizing patient needs based on inter-professional team input. The RN designs actions and interventions relevant to the patient’s culture and age. The RN determines desired outcomes in coordination with the patient and family. The RN implements patient care therapies and procedures in accordance with established policies and guidelines. The RN explains procedures and treatments to patients and families. The RN initiates actions to alleviate patient stress and discomfort. The RN recognizes and communicates changes in patient condition appropriately. The RN conducts ongoing assessments and documents responses to procedures and therapies. The RN delegates tasks appropriately and oversees team members in patient care activities. The RN monitors accuracy and completeness of delegated patient care provided by team members. The RN identifies educational needs of patients and facilitates the discharge process by executing discharge orders. The RN provides input into patient assignments based on staffing and patient acuity. The RN actively participates in unit-based activities in support of organizational values and priorities. The RN promotes a positive group dynamic and healing environment through words and actions. The RN assures nursing excellence in accordance with accepted standards and evidence-based practice. The RN participates in quality improvement activities and monitoring of documentation. The RN uses a holistic perspective regarding patient care. The unit specializes in ortho/neuro/telemetry care.
